Alcohol Withdrawal Timeline: Hour-by-Hour Symptoms & What to Expect
What happens to your body in the hours and days after your last drink — and when the danger window passes.
⚠️ Medical warning: Severe alcohol withdrawal can be life-threatening. If you experience seizures, hallucinations, severe confusion, or a fever above 38.5°C (101.3°F), go to an emergency room immediately. Do not attempt severe withdrawal at home without medical supervision.
Alcohol withdrawal begins 6–12 hours after the last drink. Symptoms peak at 24–72 hours — the highest-risk window for seizures and delirium tremens (DTs). Most acute symptoms resolve within 5–7 days. Post-Acute Withdrawal Syndrome (PAWS) can persist for weeks to months.
The Full Alcohol Withdrawal Timeline
Alcohol acts as a central nervous system (CNS) depressant, enhancing the inhibitory neurotransmitter GABA and suppressing the excitatory neurotransmitter glutamate. When heavy drinkers stop abruptly, the CNS becomes overexcited — causing the cascade of symptoms below.
The first symptoms appear in heavily dependent drinkers. These include anxiety, irritability, tremors (shaking hands), sweating, nausea, headache, and difficulty sleeping. The nervous system is beginning to rebound from alcohol's depressant effects.
Seizures can occur as early as 12 hours after the last drink. They are typically grand mal (tonic-clonic) seizures and may happen without warning. Around 2–5% of people in alcohol withdrawal experience seizures. Previous seizure history significantly increases risk. Medical supervision is critical during this window.
This is typically when symptoms are most intense. Tremors become severe, sweating is heavy, heart rate and blood pressure are elevated. Alcoholic hallucinosis — hearing, seeing, or feeling things that aren't there — can occur in about 10% of people. Unlike DTs, hallucinations at this stage occur with a clear sensorium (the person is otherwise oriented). Seizure risk remains elevated.
DTs affect approximately 3–5% of people in withdrawal and typically onset between 48 and 96 hours. Symptoms include profound confusion, agitation, fever, rapid heart rate, and unstable blood pressure alongside hallucinations. This is a medical emergency. Without treatment, the fatality rate can reach 15%. With benzodiazepine therapy and IV fluids, fatality drops below 2%. If you or someone you're with develops these symptoms, call emergency services immediately.
For most people without DTs, acute withdrawal symptoms significantly diminish by days 5–7. Tremors ease, sleep begins to stabilise, heart rate normalises. Appetite returns. The acute danger window is largely over. You may still feel anxious, tired, and emotionally flat — this is normal as the brain rebalances.
After acute withdrawal resolves, many people experience PAWS — a second, protracted phase. Symptoms include mood instability, anxiety, depression, difficulty concentrating, sleep disruption, and persistent cravings. PAWS is not medically dangerous but is the primary driver of relapse in early recovery. It typically improves week by week as the brain's neurochemistry normalises.
GABA and glutamate receptors progressively normalise over months. Sleep quality reaches baseline or better. Mood stabilises. Cravings become less frequent and intense. The risk of severe relapse episodes driven by physical craving decreases substantially by month 3–6, though psychological triggers remain. Most people feel genuinely well by 3–6 months sober.
Who Is at Highest Risk for Severe Withdrawal?
Not everyone who stops drinking will experience severe withdrawal. Risk is higher for people who:
- Drink heavily every day (8+ standard drinks per day)
- Have been drinking heavily for years, not weeks
- Have experienced alcohol withdrawal before — each episode can be more severe ("kindling effect")
- Have a history of seizures
- Are older or have other medical conditions (liver disease, heart disease)
- Have a low body weight or poor nutrition
The CIWA Scale — How Doctors Measure Withdrawal Severity
The Clinical Institute Withdrawal Assessment for Alcohol (CIWA-Ar) is the standard medical tool for measuring withdrawal severity. It scores 10 symptoms on a 0–7 scale (or 0–4 for orientation). A score below 8 indicates mild withdrawal; 8–15 moderate; above 15 severe and requiring medication.
The 10 CIWA-Ar items: nausea/vomiting, tremor, sweating, anxiety, agitation, tactile disturbances, auditory disturbances, visual disturbances, headache/fullness in head, and orientation.
Medications Used in Alcohol Withdrawal
Benzodiazepines are the gold standard for managing moderate-to-severe alcohol withdrawal. They work by enhancing GABA — the same pathway alcohol uses — preventing the CNS rebound that causes seizures and DTs. Common choices include diazepam (Valium), lorazepam (Ativan), and chlordiazepoxide (Librium).
Anticonvulsants like carbamazepine or gabapentin are sometimes used as alternatives, particularly for outpatient withdrawal with lower risk profiles. They carry less abuse potential than benzodiazepines.
Thiamine (Vitamin B1) is given to prevent Wernicke's encephalopathy — a potentially fatal neurological complication seen in people with alcohol use disorder who stop drinking. Thiamine deficiency is common in heavy drinkers due to poor diet and reduced absorption.
What Happens After Withdrawal: The Path Forward
Surviving withdrawal is the first step — staying sober is the work that follows. The brain's reward system takes months to fully normalise, which is why ongoing support dramatically improves outcomes. Options include:
- Medications: Naltrexone, acamprosate, and disulfiram are FDA-approved for alcohol use disorder and significantly reduce relapse rates when used after detox.
- Therapy: Cognitive Behavioural Therapy (CBT) and Motivational Interviewing have strong evidence bases for alcohol use disorder.
- Peer support: AA and SMART Recovery both have evidence supporting long-term sobriety.
- Recovery apps: Daily tracking, streak accountability, and AI coaching help maintain momentum through PAWS.
