Nicotine vs. Cotinine: What Tests Actually Measure
Most "nicotine tests" are actually cotinine tests. Here's why:
Nicotine has a very short half-life of approximately 2 hours. It's rapidly metabolised by the liver into cotinine. Cotinine has a half-life of 15–20 hours — about 10 times longer — making it a far more useful biomarker for tobacco exposure. By the time you take a test, nicotine itself may be largely gone, but cotinine remains at detectable levels.
The standard urine cotinine cutoff for most commercial tests is 200 ng/mL. Values above this are considered positive for tobacco use. Non-smokers typically have cotinine levels below 10 ng/mL (some background exposure from secondhand smoke is possible).
What Affects How Long Nicotine Stays in Your System
Does NRT (Patches, Gum, Vaping) Show Up on Tests?
Yes. Standard cotinine tests cannot distinguish between nicotine from cigarettes, patches, gum, lozenges, inhalers, or e-cigarettes. If you use any nicotine-containing product — including NRT — you will test positive for cotinine.

Secondhand Smoke and Passive Exposure
Heavy exposure to secondhand smoke can produce detectable cotinine levels. However, levels from passive exposure are typically far below the 200 ng/mL threshold used in most tests. Light passive exposure (being in a room where someone smoked) produces cotinine levels under 10 ng/mL in most cases — well below detection thresholds.
How Your Body Clears Nicotine: The Biochemistry
After absorption, nicotine is carried in the bloodstream to the liver, where the CYP2A6 enzyme converts approximately 70–80% of it into cotinine. Cotinine is then further metabolised (primarily into trans-3'-hydroxycotinine) or excreted unchanged in urine.
A small amount of nicotine is also converted directly to nicotine N-oxide by a different enzyme (FMO3). The kidneys excrete both cotinine and its metabolites, so good hydration and kidney function speed this process.
There is no supplement, detox drink, or product that meaningfully accelerates this metabolic pathway. The only reliable way to clear nicotine faster is to stop using it — then let time and hydration do their work.
