The Short High Drives Compulsive Bingeing

Cocaine's high lasts only 20–90 minutes depending on route, far shorter than the craving cycle it creates. Users redose every 20–60 minutes during a binge, and each binge depletes dopamine further — deepening the crash that follows.

Unlike opioids (which have MAT options) or benzos (which have established taper protocols), there is no FDA-approved medication for cocaine use disorder. This makes behavioral treatment — particularly Contingency Management — the primary evidence-based approach and one that actually outperforms the medication-plus-counseling model used for other drugs when well-implemented.

Evidence-Based Treatment

Contingency Management (CM)

Rewards confirmed abstinence with vouchers or prizes. Multiple RCTs show CM produces significantly better cocaine abstinence rates than control conditions — the most consistently effective intervention for stimulant use disorder.

Cognitive Behavioral Therapy (CBT)

Identifies triggers, builds coping skills, and restructures use-supporting thought patterns. Carroll et al. (1994) showed CBT benefits strengthen post-treatment — patients keep applying skills after formal sessions end.

Topiramate / NAC (emerging)

Topiramate reduced cocaine use in a 2004 RCT (Kampman). N-Acetyl Cysteine may reduce cravings via glutamate homeostasis. Both are used adjunctively in some programs alongside behavioral treatment.
